If you have an admin request for the surgery such as blood test results, a sick note, cancelling an appointment or needing your patient record you can submit a request under 'Any other question'
Your practice may also have some custom buttons on their 'New request' selection screen, allowing you to instantly fill in a form, such as a sick note, or medication request. If they have these options available, this is a faster and more effective route to submitting your request through to them:
